Testosterone is mainly present in bound form, but only the unbound form is biologically active. The free androgen index (FAI) indicates the ratio of testosterone to SHBG. It provides an indication of possible relative hormonal deficiency or relative hormonal excess. The formula for calculation is:

Total testosterone x 100 / SHBG
